Nurses:
We are currently seeking Registered Nurses (RNs) and Registered Practical Nurses (RPNs) to join our growing team. As a team member you will provide safe and holistic nursing care to patients in a variety of service delivery locations across Greater Toronto Area.
Responsibilities
In collaboration with the patient/family/caregiver and other health care professionals, help formulate a plan of care based on patient assessment.
- Provide nursing care to patients according to the written plan of care.
- Evaluate the effectiveness of treatment plan(s) and adjust accordingly in consultation with the patient/family/caregiver and other health care
professionals - Act as a patient advocate by protecting and promoting patient rights.
- Promote patient safety and assist them in living comfortably in their home environment.

Personal Support Workers
We are continually seeking compassionate care givers to join our team. You will play a vital role in providing personal care and support clients with their activities of daily living to lead a better life.
Responsibilities
- Provide personal care and assistance with activities of daily living (bathing, personal hygiene, skincare, light meal preparation, feeding etc.) in client’s home.
- Continually assess, anticipate, identify, and respond to new and
changing needs of clients as appropriate. - Perform delegated and/or assigned tasks under the supervision of a Home Support Supervisor.
- Staff are required to travel within assigned region between clients’ homes.
Qualifications
- Valid Personal Support Worker Certificate from Ministry of Training, Colleges, and Universities or the Ministry of Education approved program.
- RN/RPN Student who with a minimum successful completion of
year one of the program. - Internationally licensed Nurse, Midwife or Physician with their
OPSWA registration. - Physical ability to meet the demands of the job (ex: ability to
stand, bend, walk, crouch, kneel and reach freely for extended periods, ability to lift 23 kg independently, ability to assist clients with standing, walking, sitting; able to reposition clients in bed). - Demonstrated ability to provide exceptional personal care and customer service.
- Valid Standard First Aid and Level C CPR.
- Valid Vulnerable Sector Criminal Check.
- Updated immunization record including TB Test (last 6 months).
- Fully vaccinated for Covid-19.
